New Patients
We understand that patients can feel nervous about visiting a dentist, which is why at Havenview Dental, we accommodate all new and returning patients with a warm and welcoming atmosphere, greeted by our caring and friendly team, as soon as they enter our comfortable facility.
We ensure all our patients are well prepared for their first appointment by providing a complete spectrum of our offered services and working with you to fill out our new patient forms prior to your first visit, to make it easy and quick when you visit us. Our purpose is to cover all of the patient’s needs with a suitable assessment while being efficient in a respectable amount of time.
For your first appointment, please take a few minutes to complete the Medical History Form & New Patient Form. When completed, you may bring them in or email it back to us. If you have been treated by another dental professional within the last 3 years please fill out, print, and return the Information and X-ray Release Form to us so we can transfer your records as soon as possible. By doing so, we will be ensured there won’t be any duplications of the records at your first appointment and that we have the most up to date information needed to begin our journey in helping you reach the optimal goal of having oral hygiene and healthy white teeth.
